NEW! UBEditorPro.com now includes the UB-04 claim form and data specifications

UBEditorPro.com now includes the UB-04 claim form and data specifications (v 1.00) as released by the National Uniform Billing Committee (NUBC) and approved by the Office of Management and Budget (OMB) on August 28, 2006.

The UB-04 replaces the current UB-92 billing form and will be used by hospitals and other institutional providers to bill government (Medicare, Medicaid, etc.) and commercial health plans. The new claim form will be phased in over a transition period from March 1, 2007 to May 22, 2007. It is to be used exclusively for institutional billing beginning May 23, 2007. The UB-04 data set accommodates the National Provider Identifier (NPI) and incorporates a number of other important changes and improvements.

This feature provides a listing of all official UB-04 claim form locators, their descriptions and usage guidelines. Coding and billing tips are added to each data element throughout the transition period and beyond.

This crosswalk tool provides users with an easy way to become familiar with the UB-04 FLs, data elements and content requirements needed to transition from the UB-92 to new UB-04 claim form.
